Declaration

As an authorised representative on behalf of the company, I hereby declare and agree to be bound by all Terms & Conditions listed below:

1. That I will abide by the FPA Australia Memorandum and Articles of Association/Constitution and the Terms & Conditions outlined at www.fpaa.com.au/membership

2. That the entity will be bound by FPA Australia's most current Code of Practice for the entire membership period.  The current Code of Practice can be found at www.fpaa.com.au/membership

3. That the entity holds public & product liability insurance for a minimum of $10million, the policy covers all business activities and the policy will be maintained and current for the period of membership 

4. That all information provided is true and correct
